Terms of Service
These Terms of Service constitute a legally binding agreement between you (the Client, Patient, or Authorized Representative) and Pondfork Transportation, LLC regarding the provision of our non-emergency medical transportation services and related scheduling support.
1. Provision of Transportation Services
Pondfork Transportation, LLC agrees to provide non-emergency medical transportation (including ambulatory, wheelchair, and stretcher transport where offered) as described in your booking confirmation, written quote, or service agreement. We retain discretion over routing, vehicle assignment, and operational methods necessary to complete trips safely and efficiently, subject to applicable laws and regulations.
2. Scheduling, Cancellations, and Client Responsibilities
Pickup and arrival times are estimates based on traffic, weather, and prior trips. Accurate pickup addresses, mobility information, and reachable contact numbers are required. Cancellations should be provided as early as possible; late cancellations or no-shows may incur fees as stated in your booking confirmation. Delays caused by incomplete information, unavailable patients at pickup, or facility hold times may affect scheduled windows without penalty to Pondfork Transportation, LLC.
3. Fees and Payment
Client agrees to pay trip fees, wait-time charges, and any applicable surcharges according to the rate schedule or invoice provided at booking. Private pay, facility billing, and insurance arrangements vary by contract. Unless otherwise agreed in writing, payment is due at the time of service or within the terms stated on your invoice. Pondfork Transportation, LLC does not provide emergency ambulance services; life-threatening situations require calling 911.
4. Service Duration and Termination
Recurring transportation may be arranged on an ongoing basis until either party provides reasonable notice to modify or cancel the schedule. Either party may discontinue future scheduled trips with written or documented notice as specified in your service agreement. Upon termination, outstanding balances remain due for completed trips.
5. Limitation of Liability
To the fullest extent permitted by law, Pondfork Transportation, LLC shall not be liable for indirect, consequential, punitive, or special damages arising from use of our services. Our maximum aggregate liability for any claim relating directly to transportation services rendered shall not exceed the total fees paid to Pondfork Transportation, LLC for the specific trip giving rise to the claim, except where liability cannot be limited under applicable law.
